Running campaigns for treatment centers, we see a detox or residential admission from Google Ads cost $2,000 to $5,000 in-network and $7,000 to $15,000 out-of-network. A VOB (verification of benefits) costs from a couple hundred dollars up to $2,000 in some out-of-network cases. Before you spend a dollar, you need LegitScript certification and Google's own approval for addiction services.
Local Services Ads are not an option, because Google's LSA category list has no addiction treatment category. That leaves Search as the main channel.
This guide is for owners and marketing leads at treatment centers who are about to launch Google Ads, or who run them now and want to know why the numbers look the way they do. Every policy, fee and benchmark below links to its source.
Step one: get approved before you build anything
Google restricts the promotion of recovery-oriented drug and alcohol addiction services. The policy covers clinical treatment providers, sober living and mutual help groups, lead generators and referral agencies, and crisis hotlines. Approved advertisers can run these ads in six countries: Australia, Canada, France, Ireland, New Zealand and the United States.
In the US, Google requires two approvals. Its healthcare policy says you "must be certified as an addiction services provider by the LegitScript Certification program" and that "Advertisers must also be certified with Google." You file the Google application at the child account level, not the manager account, with your Google Ads customer ID and the certified domain.
What LegitScript costs
| Fee | Standard (1 to 9 facilities) | Individual practitioner |
|---|---|---|
| Application fee, per facility | $1,595 | $535 |
| Annual certification fee, per facility | $3,095 | $1,070 |
| Expedited review (optional) | $2,500 per application | |
Source: LegitScript's Addiction Treatment Certification FAQ. The practitioner tier is for practices with three or fewer client-facing practitioners at one location. The expedite fee gets your review started within 2 business days. LegitScript does not publish a standard review time.
Our math: a single facility on the standard tier pays $1,595 + $3,095 = $4,690 in year one before any ad spend. With the expedite fee, that becomes $7,190.
Not everyone qualifies. LegitScript lists sober living homes, lead generation websites and assessment-only services as generally not eligible. Telehealth SUD providers can qualify. For the full process, read our LegitScript compliance guide.
What about mental health only? Google's addiction policy names drug and alcohol addiction services. It says services for "behavioral addiction, or nicotine addiction" are out of scope. If you offer any SUD care, plan on certification.
Why the rules exist: in 2017 Google restricted rehab ads after it "found a number of misleading experiences among rehabilitation treatment centers." It brought them back in 2018, only for advertisers assessed by LegitScript.
Can rehabs use Local Services Ads?
Google says Local Services Ads "are available only for certain service categories." We read the full US list. It has no addiction, rehab, substance use or mental health category. The only therapist categories are massage, occupational and physical therapist. Skip LSA and put the budget into Search.
What does Google Ads cost a rehab?
No single public number tells you your cost. Here is what each source measures, side by side.
| Metric | Number | Source |
|---|---|---|
| CPC, "detox near me" | $95.56 | Our Keyword Planner pull, US, Oct 4, 2026 |
| CPC, "inpatient rehab" | $57.27 | Same pull |
| CPC, "iop near me" | $52.95 | Same pull |
| CPC, "rehab centers near me" | $44.85 | Same pull |
| Median CPC, addiction recovery campaigns | $3.46 | LocaliQ |
| Median cost per lead, addiction recovery | $120.30 | LocaliQ |
| Cost per lead, paid search | $406 | Webserv 2025 |
| Cost per VOB, paid search | $2,552 | Webserv 2025 |
| Cost per admit, paid search | $16,608 | Webserv 2025 |
How to read it:
- Keyword Planner estimates are for head terms. It returned no data for "drug rehab near me," "alcohol rehab near me" or "rehab near me." SEMrush shows lower estimates on some of the same terms, such as $34.30 for "detox near me."
- LocaliQ reports medians from 3,542 US search campaigns that ran from October 1, 2024 to September 30, 2025, with at least 43 campaigns per subcategory. Addiction recovery had the lowest median conversion rate in its table, 0.50%, down 87% year over year. Our read: a median across many small advertisers says little about what a head-term click costs in a busy metro.
- Webserv is an agency. It pooled $16.5M of paid search spend across its own clients in 2025. No third party audited it. It is still the only public dataset we found that reports cost per VOB.
Our math: from click to VOB
Cost per VOB = CPC ÷ click to inquiry rate ÷ inquiry to VOB rate. An inquiry is a call or form fill.
- CPC, $45: "rehab centers near me" from our Keyword Planner pull.
- Click to inquiry, 8%: LocaliQ's 8.09% average for all healthcare search ads. Assumption: your account converts like healthcare overall, which is far above LocaliQ's 0.50% addiction median.
- Inquiry to VOB, 25% to 40%: the range Webtonic, an agency, cites for healthy accounts.
$45 ÷ 0.08 = $563 per inquiry. $563 ÷ 0.40 = $1,406 per VOB, and $563 ÷ 0.25 = $2,250 per VOB. That lines up with what we see in campaigns. At a $95 detox click, the same rates give $2,969 to $4,750 per VOB, above the top of our range. Our read: accounts that land inside our range either convert clicks to calls at better than 8% or pay less per click than the Keyword Planner estimate. Track your own rates before you trust any benchmark. For the full cost per admission math, see our rehab cost per admission guide.
Targeting limits most rehabs miss
Google treats health as a sensitive interest category for personalized ads. Its examples include "Counseling services for mental health issues like depression, anxiety, and addiction." Advertisers in sensitive categories "can't use advertiser-curated audiences." That list includes:
- Customer Match (your uploaded patient or lead lists)
- Your data segments (remarketing lists of site visitors)
- Lookalike segments and audience expansion
Google's predefined audiences, such as in-market and affinity segments, demographics and location, stay available. Google also warns that Demand Gen campaigns use advertiser-curated audiences by default and may be restricted. The practical result: a rehab cannot build its Google plan on remarketing. Search is where the intent is.
The keyword groups that matter
High intent (bid here first)
- "Rehab near me" and "rehab centers near me"
- "Inpatient rehab [city]" and "residential rehab [state]"
- "Detox [city]" and "medical detox [state]"
- "IOP [city]" and "PHP [city]"
- Insurance terms: "rehab that takes [carrier]"
- Substance terms: "alcohol rehab [city]," "opioid treatment [city]"
Mixed intent (test with care)
- "Best rehab [state]"
- "How much does rehab cost"
- "How to get someone into rehab," which family members search
Research and job queries (add as negatives)
- "Signs of addiction," "what is rehab like," "how long does rehab take"
- "AA meetings near me," unless you host meetings
- "Free rehab," unless you offer it
- "Rehab jobs," "counselor jobs," "internship"
Pull the search terms report every week and add anything off target to your negatives.
Campaign structure
Split campaigns by intent and level of care so you control budget and message for each.
- Brand: your center's name and variations.
- Local high intent: "near me" and city terms, with ad groups by city.
- Level of care: detox, residential, PHP and IOP in separate campaigns. Our Keyword Planner pull shows "detox near me" at $95.56 against $52.95 for "iop near me," so a shared budget can let detox eat the IOP spend.
- Insurance: carrier terms, sent to pages that name the plans you accept.
- Family: "how to help someone with addiction" and similar terms, with messaging for the person making the call.
Landing pages and calls
Send each campaign to a page built for it, not your homepage. What the public data says:
- Calls matter. In Ruler Analytics' data, 37.2% of health and social care conversions were calls. Put the phone number in the first screen and use call tracking so you know which keyword produced which call.
- Plain language converts. In Unbounce's health and wellness data, pages written at a 5th to 7th grade reading level had a 10.8% median conversion rate. The median for its healthcare pages overall was 5.1%.
- Design for phones. The same Unbounce report says "Mobile brings 7x more traffic but desktop converts 22% better." Test the page on a phone first.
Match the page headline to the ad, name the insurance you accept near the top, and show your real staff.
Count VOBs, not form fills
Google bids toward whatever conversion you tell it to count. If a form fill is the goal, you will get form fills. Send VOBs and admissions back instead. Google's enhanced conversions for leads lets you import offline conversions matched to the original click. Google says that starting June 15, 2026, these uploads move to its Data Manager API.
Patient data is sensitive. Have your compliance counsel approve what you send to any ad platform before you turn it on.

- Certification: LegitScript publishes no standard review time. Paying the $2,500 expedite fee starts the review within 2 business days. The Google approval comes after that.
- Ad review: Google says "Most ads are reviewed within one business day."
- Bid learning: after a change to automated bidding, Google says it "can take a few conversion cycles (1-2 typically)" to calibrate. If a VOB takes days to confirm, plan on weeks, not days.
A healthy rehab Google Ads account

Frequently asked questions
How much do Google Ads cost for a rehab center?
In our experience, a detox or residential admission from Google Ads costs $2,000 to $5,000 in-network and $7,000 to $15,000 out-of-network, and a VOB costs from a couple hundred dollars up to $2,000. Our Keyword Planner pull on October 4, 2026 showed $95.56 per click for "detox near me" and $44.85 for "rehab centers near me." Webserv's 2025 agency dataset reports $2,552 per VOB and $16,608 per admit from paid search.
Do I need LegitScript to run Google Ads?
Yes, for drug and alcohol addiction services in the US. Google requires LegitScript certification plus its own Google certification. LegitScript's standard fees are $1,595 to apply and $3,095 a year per facility. Google's policy says behavioral addiction and nicotine addiction services are out of scope. Read the LegitScript compliance guide for the full process.
Can a rehab use Google Local Services Ads?
No. Google offers Local Services Ads only for listed categories, and the US list has no addiction, rehab, substance use or mental health category. Use Google Search ads instead.
Can a rehab use remarketing or Customer Match on Google?
No. Google treats addiction counseling as a sensitive health category for personalized ads. Advertisers in that category can't use advertiser-curated audiences, which include Customer Match, your data segments (remarketing), lookalike segments and audience expansion. Predefined Google audiences and location targeting are still allowed.
How long until I see admissions from Google Ads?
Approval comes first. LegitScript publishes no standard review time, and a $2,500 expedite fee starts the review within 2 business days. After that, Google reviews most ads within one business day. Automated bidding then needs a few conversion cycles to calibrate, according to Google, so a center that counts VOBs should plan on weeks before the numbers settle.
What is the biggest mistake rehabs make with Google Ads?
Counting form fills as the goal. Google bids toward the conversion you give it, so form fills bring more form fills. Track cost per VOB, send VOBs and admissions back to Google Ads after a compliance review, and add research queries like "signs of addiction" to your negative keywords.
